About this file
This is a Disqualification Disclosure form required by the State of California under Public Contract Code Section 10162 for prospective bidders on the City of Pasadena's On-Call Telecommunications Facilities Entitlement Case Assessment and Management Services contract. The form requires bidders to disclose, under penalty of perjury, whether the bidder, any officer of the bidder, or any employee with a proprietary interest in the bidder has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to a violation of law or safety regulation. Bidders must check either "Yes" or "No" and provide an explanation with supporting documentation if the answer is affirmative.
This mandatory disclosure document serves as a qualification screening mechanism to ensure that only contractors with satisfactory compliance histories are eligible to compete for the telecommunications facilities permitting and project management services. The form establishes a baseline integrity requirement for all bidders and demonstrates the City of Pasadena's commitment to contractor accountability and regulatory compliance in the procurement process.
