About this file

This is a Disqualification Disclosure form required by the City of Pasadena, California, in accordance with California Public Contract Code Section 10162, for prospective bidders to the Low-Interest Solar and Energy Storage Loan Program. The form requires bidders to disclose, under penalty of perjury, whether the bidder, any officer of the bidder, or any employee with proprietary interest has ever been disqualified, removed, or otherwise prevented from bidding on or completing a federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate their response by checking either "Yes" or "No" on the form.

If a bidder answers affirmatively to the disqualification question, they are required to provide a detailed explanation of the circumstances and attach supporting documentation to the disclosure form. This disclosure requirement is a standard compliance measure designed to ensure that only qualified entities with clean regulatory and legal histories are permitted to participate in government contracting opportunities related to the City of Pasadena's solar and energy storage loan initiative.
