About this file

This document is a Design-Build Team Reference Check Exhibit from the City of Phoenix Office of the City Engineer's Design and Construction Procurement section, detailing a reference evaluation process for potential design-build contractors. The exhibit provides comprehensive instructions for completing a performance evaluation form, which will be used to assess and score design-build teams competing for a contract (RFx number 600000XXXX). The process involves submitting up to three reference checks, with each reference potentially earning up to 7 points (for a total of 21 possible points), and requires references to be submitted via email to soq.referencechecks@phoenix.gov by a specified date and time.

The reference evaluation form requires scoring across six key performance categories, including business relationship, team consistency, problem-solving, project completion, work quality, and innovation. Evaluators will rate each category on a scale from 1 (Unsatisfactory) to 4 (Excellent), with points awarded accordingly. Additionally, references are asked whether they would be willing to contract with the firm again, which can earn an extra point. The completed form becomes a public record and will be released upon request, emphasizing transparency in the procurement process. The evaluation aims to provide a comprehensive assessment of a design-build team's past performance to inform contract award decisions.
