About this file

This document is a Government-Wide Debarment & Suspension Certification form related to the Wildcat Creek Pump Station Rehabilitation Construction Services project for the City of Rock Hill in South Carolina. The certification requires the contractor to attest that their principals are not currently debarred, suspended, or excluded from government transactions, and have not been convicted of or charged with specific criminal offenses within the past three years.

The document mandates that the contractor certify they have not been involved in fraudulent activities, criminal offenses, or public transaction violations that would disqualify them from bidding on government contracts. If unable to fully certify the statements, the contractor must provide an explanation. The form requires a signature from an authorized official, along with a witness signature and date, to validate the truthfulness and accuracy of the provided information in compliance with 31 U.S.C. Sections 3801 et seq.
