About this file
This is a Debarment Certification Form AD-1048 issued by the U.S. Department of Agriculture, required for lower tier covered transactions under Executive Order 12549. The form applies to the Fire Flights contract opportunity in Oklahoma, which involves a private vendor providing fixed-wing aircraft for wildfire detection within the Forestry Services' Eastern Areas. The certification requires prospective participants to confirm that neither they nor their principals are currently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from participation in federal transactions by any Federal department or agency.
Organizations submitting proposals must complete the certification form by providing their organization name, award or project number, authorized representative information, and signatures with dates. If an organization cannot certify to the statements contained in the form, it must attach an explanation to its proposal. The form constitutes a material representation of fact, and knowingly providing erroneous certification may result in suspension, debarment, or other remedies pursued by the originating federal agency. Participants agree to provide immediate written notice if their certification becomes erroneous and commit to including this certification requirement in all lower tier covered transactions and related solicitations.
