About this file

This is a Contract Summary document for Workers' Compensation IME Scheduling Services issued by the Arizona Department of Administration - State Procurement Office (ADOA-SPO). The contract involves seven contractors: Alliance Evaluation Services, CorVel IME Corporation, ExamWorks, Integrated Medical Evaluations, Medical Evaluation Specialists, QTC Commercial Services, and Woodlake Medical Management. The contract is a statewide and cooperative agreement available to the State of Arizona and co-op agencies, with a 2% fee for cooperative purchases.

The contract provides Independent Medical Examination (IME) scheduling services, effective from 11/3/2025 to 10/31/2030, with an initial one-year term and four potential one-year renewals. Users can select from multiple awarded suppliers based on their specific needs, with pricing determined by individual contract pricing sheets for each contractor. Purchase orders can be issued through APP (presumably a procurement system) by referencing the standard contract release item and matching invoice pricing. The contract was competitively awarded and includes provisions for security controls, with all suppliers completing a baseline infrastructure security assessment and required to complete additional security spreadsheets prior to service delivery.
