About this file

This document is an Offer Response Form for an Automotive Parts and Repair contract for Medium and Heavy Duty Vehicles (BPM006654), focused on the state of Arizona. The contract is offered by a vendor (likely a dealership) authorized for General Motors and ISUZU Truck, covering multiple counties including La Paz, Maricopa, and Pima. The contractor provides statewide coverage and offers additional services such as towing.

The contract encompasses three primary categories: OEM Parts, OEM Labor and Repair, and Labor and Repair with non-OEM parts. Pricing includes a 10-20% markup over dealer cost for parts, with a base OEM labor rate of $145 per hour. The contractor offers extensive service options including auto body repair with specific hourly rates for frame, paint, mechanical, and aluminum work. Delivery charges are set at $10 per county, with a $20 flat rate for pickup/delivery services. Warranties include two years/unlimited mileage for GM dealer-installed parts and 12 months/12,000 miles for Isuzu parts. No purchasing card surcharges are applied, and the contract includes provisions for core part returns and potential restocking fees.
