About this file

This document is a Certificate of Liability Insurance and Workers' Compensation Policy Endorsement issued by Indemnity Insurance Company of North America for ADL Solutions, located in Chandler, AZ. The certificate and endorsements are for a contract with the State of Arizona and Department of Economic Security (Division ADES Contract ID CTR077216), with Lockton Companies, LLC serving as the insurance producer. The policy covers the period from 10/01/2025 to 10/01/2026 and includes provisions for alternate employer coverage and waiver of subrogation in favor of the State of Arizona and its departments, agencies, boards, commissions, universities, officers, officials, agents, and employees.

The policy provides workers' compensation and employers' liability insurance with coverage limits of $1,000,000 for each accident, each employee, and policy limit. The endorsements include an Alternate Employer Endorsement and a Waiver of Right to Recover from Others Endorsement, which protect the State of Arizona by limiting the insurance company's subrogation rights. The policy specifically notes exceptions for certain states like AK, HI, MI, OK, TX, KY, NH, and NJ, and includes special provisions for states like CA, UT, TX, and MO regarding subrogation and contract limitations.
