This document is a comprehensive insurance policy package issued by Great American Insurance Company to EPI-HAB Phoenix, Inc., covering commercial general liability, automobile liability, and workers' compensation for the period of October 9, 2024 to October 9, 2025. The policy specifically names the State of Arizona/Arizona Office of Tourism as an additional insured, with coverage extending to the state's departments, agencies, boards, commissions, universities, officers, officials, agents, and employees.

It covers a leased 2019 Ford F550 vehicle with comprehensive and collision deductibles of $1,000. The policy includes various endorsements that broaden coverage, such as additional insured protections, waivers of subrogation, and extensions for mental anguish, property damage, and newly acquired vehicles. The insurance is designed to protect against liability arising from EPI-HAB Phoenix's business operations and activities performed on behalf of the State of Arizona.
