About this file
This document is a Contract Amendment Summary issued by the Arizona Department of Transportation (ADOT) Procurement division, specifically amending a contract with Southwest Slurry Seal Inc. for Slurry and Micro Surfacing Services. The amendment is designated as Amendment Number One (1) and follows the Uniform Terms and Conditions, Section 5, regarding Contract Changes.
The amendment modifies two specific sections of the contract's specifications related to equipment requirements. In Section 4.10, concerning Slurry Seal Mixing Equipment, the amendment mandates that machines must be continuous-run machines capable of loading materials while applying slurry seals, with truck-mounted mixing machines prohibited unless specifically approved by the Department. Similarly, in the Micro-Surfacing Mixing Equipment section, the amendment restricts the use of truck-mounted mixing machines, which are not permitted without explicit departmental approval. All other terms, conditions, and provisions of the original contract remain unchanged.
