About this file

This is a Contract Amendment Summary issued by the Arizona Department of Transportation (ADOT) in Phoenix, Arizona, amending a contract with Superior Supply Inc. for Asphalt Cold Patch Materials. The document is Amendment Number One (1) and was revised in April 2020 by ADOT's Procurement division.

The amendment modifies two key sections of the original contract: Specifications and Special Terms and Conditions. Changes include requirements for certified weigh tickets with gross, tare, and net weight for each delivered load, and revised shipping terms specifying F.O.B. Destination delivery. A new line item (CTR070196-3: Freight) was added to the price list, and the amendment clarifies that freight costs will be paid by ADOT as a pass-through cost without markup. The contractor must include estimated freight costs in quotes and provide a copy of the freight invoice after delivery. All other terms and conditions of the original contract remain unchanged.
