About this file

This document is Amendment No. 1 to Contract No. CTR066001 for Substance Use Disorder Treatment, issued by the Arizona Department of Corrections, Rehabilitation & Reentry (ADCRR) and involving SAGE Counseling as the contractor. The amendment modifies an existing contract for substance use disorder treatment services in various Arizona state correctional facilities.

The amendment extends the contract term through July 15, 2025, and introduces a new delivery method for Outpatient substance use disorder treatment consisting of 15 cumulative hours of treatment per offender through weekly 1-hour group sessions. The contract covers multiple facilities including ASPC-Safford/Ft. Grant, ASPC-Yuma, ASPC-Eyman, ASPC-Lewis, ASPC-Perryville, ASPC-Phoenix, ASPC-Safford, ASPC-Tucson, and Maricopa Reentry Center. The fixed fee rate for these 1-hour Outpatient group treatment sessions is $325 per session across weekday day hours, weekday evening hours, and Saturday hours. The amendment also allows for certain services to be performed virtually with prior ADCRR approval and requires the contractor to provide a minimum of three cohort groups per week across four treatment types: Moderate, Intensive, DUI, and Outpatient.

Acknowledgement and Authorization This amendment shall be fully executed upon Contractor’s signature and attachment in the Arizona

Procurement Portal by the Procurement Officer

In accordance with Uniform Terms and Conditions section 5, Contract Changes, contract

CTR066001 is amended as follows:

1. The contract term is extended for an additional twelve (12) months through July 15, 2025.

2. A new delivery method for Outpatient substance use disorder treatment services is added for each contracted location. This delivery method is intended to consist of a cumulative fifteen

(15) hours of treatment for each participating offender, conducted in weekly 1-hour Outpatient group sessions. Treatment shall be consistent with the other contracted services, and will also use the “Living in Balance” materials described in the contract. As with the other substance use disorder treatment services, ADCRR will provide the following:

The facilitator’s manual for each treatment provider

Books/workbooks required for treatment for each treatment participant (hardcopy or digital)

A laptop computer for each counselor that will be available while at the facility

All necessary safety equipment that is required at the facility they are providing treatment at (e.g., stab vest, radio, etc.)

Physical space accommodations and furniture needed for sessions at prison facility

Cohort groups under this new delivery method will be at ADCRR’s discretion, and may not begin without prior ADCRR written approval. As applicable, the modifications in paragraph (3.) below may apply to these sessions if approved in writing at ADCRR’s discretion.

The fixed rates for the weekly 1-hour Outpatient sessions are shown in Attachment A to this

Amendment. These line items shall be added to the online APP contract price list after full execution of this Amendment.

3. The requirement for the functions listed below to be performed onsite and in-person at the applicable ADCRR facility location is modified to allow for these services to be performed virtually, but only upon prior written approval by ADCRR. At ADCRR’s discretion, such approval may be modified or retracted upon written notice from ADCRR at any time.

4. Contractor shall provide, for each contracted facility, treatment for a minimum of three (3) cohort groups per week, which may consist of any combination of the four (4) treatment types under this contract:

Moderate - four sessions per week, each for a minimum 2-hour duration

Intensive - four sessions per week, each for a minimum 2-hour duration

DUI - two sessions per week, each for a minimum 2-hour duration

Outpatient – one session per week for a minimum 1-hour duration

For example, two (2) Moderate treatment cohort groups (each meeting four times per week) and one (1) Outpatient cohort group (meeting once per week) would fulfill this requirement.

The effective date for compliance with this provision shall be three (3) months from the date of full execution of this Amendment.

Notwithstanding the foregoing, Contractor will not be held responsible for noncompliance(s) in meeting the required cohort group sessions per week during any time periods in which ADCRR decisions or obstructions are the proximate cause of such noncompliance; such proximate causes may include such things as non-approval for a cohort to begin due to a lack of an adequate number of inmate participants, a lack of necessary resources to be provided by

ADCRR per contract terms, a lack of ability for Contractor to provide services virtually to any facilities for which it has not already provided services under this contract, or discretionary decisions made by ADCRR which prevent cohort sessions from occurring when Contractor is otherwise able and willing to comply. However, in any such instances described above, once

ADCRR’s proximate cause has been cured, or mitigated to a mutually acceptable degree, Contractor is responsible to come into compliance as soon as possible, but in any case in no more than three (3) months from the time of such cure or mitigation.

5. Contractor is required to use the NaphCare/TechCare Electronic Health Record (EHR) System upon completion of training provided by ADCRR. Contractor shall make its applicable personnel available for such training at ADCRR’s direction. The EHR software will be provided at no cost to

Contractor.

All other requirements, specifications, terms and conditions remain unchanged.
