Group Coverage for Scheduled Treatment Sessions:

In order to maintain scheduled treatment sessions when our regular staff members are unable to facilitate their assigned groups, SAGE will train additional managers, supervisors, and outpatient counselors to address the situation. If a group can be provided via telehealth services, one of our trained outpatient staff will typically be able to accommodate groups virtually, as needed. Coverage for in-person groups would be based on the location and availability of appropriate staff and therefore more limited.

Expanding to new sites is contingent upon our ability to hire and train clinical staff to fill the positions. We are not committing to be able to fill all those positions, but if we are awarded a contract, we will immediately implement our plan to develop the additional capacity to do so. We expect that being able to provide more of the services through telehealth will allow us to add counselors more quickly than in the past.

We have a strategy to recruit more staff by offering higher compensation packages along with lower productivity requirements than we have for our outpatient clinicians.

Additionally, we may have staff travel greater distances to reach these outlying prisons, if no virtual treatment options are available. Consequently, we slightly increased our compensation rates to enable us to cover those additional expenses.

Our hope is that the combination of a consistent, aggressive recruitment approach with an enhanced compensation package will allow us to gradually fill a large number of positions around the state for this program.

4.6.2. The selected Offeror(s) must provide at least one of the substance use disorder treatment services shown below, depending on the facility's needs. Historically, ADCRR's needs have been approximately 90% for moderate treatment, and approximately 10% for intensive treatment.

SAGE understands that we must provide at least one of the substance use disorder treatment services shown below, and that approximately 10% of those services are for intensive treatment. We propose to provide all three of the services: Moderate, Intensive, and DUI. SAGE Counseling has read, understands, and shall comply with Sections and Subsections identified herein.

4.6.2.1. Moderate Treatment - designated as a cumulative one hundred (100) hours of treatment for each participating inmate.

SAGE proposes to provide Moderate Treatment. We have been providing

Moderate Treatment under the current contract for this program, and we also provided it in the Perryville Prison for several years using Dr. Najavits’ curriculum, along with Dr. Covington’s curriculum. We have read, Substance Use Disorder Treatment

4.6.2.2. Intensive Treatment - designated as a cumulative two hundred (200) hours of treatment for each participating inmate.

SAGE proposes to provide Intensive Treatment, and we have experience providing this service under this current contract. We have also provided intensive treatment for ADCRR in the Perryville Prison for several years using

Dr. Najavits’ curriculum along with Dr. Covington’s program. We have read, understand, and shall comply with Sections and Subsections identified herein.

4.6.2.1. DUI Education and Treatment Services. These services must meet all requirements of Arizona Administrative Code (A.A.C.) Title 9, Chapter 20, Article 1, DUI Services. Contractor will utilize the Flex

Modules by Change Companies, and Living in Balance Curriculum by Hazelden.

SAGE proposes to provide DUI Education and Treatment Services that will comply with all requirements of A.A.C, Title 9, Chapter 20, Article 1, DUI services. We agree to utilize the Flex Modules by the Change Companies and

Living in Balance curricula by Hazelden.

We have provided DUI Screening, Education, and Treatment for ADCRR for

5 years in the Perryville prison. We have also provided those services for 25 years in our multiple outpatient offices. SAGE has been licensed as a full-service DUI provider by ADHS since we opened the agency. We have read, understand, and shall comply with Sections and Subsections identified herein.

4.6.3. Treatment shall be delivered in accordance with the following guidelines:

4.6.3.1. Group sessions with minimum two (2) hour duration.

4.6.3.1.1. Face-to-face in person - minimum of fifteen (15) inmates.

We agree to provide treatment in 2-hour sessions as we are presently doing in the prisons. For in-person groups, we understand there will be a minimum

We agree to provide treatment in 2-hour sessions as we are presently doing in the prisons. For virtual groups, we understand there will be a minimum of 12 inmates. SAGE Counseling has read, understands, and shall comply with

4.6.3.2. If ADCRR is unable to fill a group, the group can be started with fewer than the minimum number of inmates if approved by ADCRR's

Counseling and Treatment Service Administrator.

SAGE understands that if ADCRR is unable to fill a group, it may be started with fewer than the minimum number of inmates/clients, if approved by ADCRR. SAGE has read, understands, and shall comply with Sections and

4.6.4. Offeror shall submit with their proposal a detailed plan of the services they can provide. The plan shall clearly indicate:

We created the chart below to provide a detailed plan of the services we propose to provide under this contract. The chart summarizes services we are already providing, additional services that we expect to implement soon, and more services we plan to add after we hire additional clinicians.

As we are currently providing treatment under this contract, we list the prisons where we are currently providing services and the information that is requested in this section, specifically in which prisons we want to serve, the offered groups, the group types, and SAGE’s plans to maintain scheduled groups even when our originally scheduled counselor is unavailable.

Expanding to new sites is contingent upon our ability to hire and retain clinical staff to fill the needed positions, so we are not committing to be able to fill all those positions. But if we are awarded a contract, we will immediately implement our plan to develop the additional capacity to do so and stay focused on adding additional clinical staff. We will likely be able to add more services sooner in prisons where we can provide some telehealth services.

As essential aspect of our assessments is the collaborative formation of the client’s initial Individualized Service Plan (ISP). SAGE always completes a comprehensive Individual Service Plan (ISP) for each client before s/he begins treatment. The ISP is one of the most important components of the assessment process, as it provides a roadmap of treatment interventions, goals, objectives, and interventions that our clinicians and clients use to guide treatment and measure progress.

The Individualized Service Plan is primarily generated from the client’s point of view, with guidance from the clinician. They jointly formulate the various components of the ISP, including presenting issues, goal statements, multiple objectives based on the goals, and specific interventions and supports based on the objectives. We typically also document the duration and intensity of treatment, participation expectations, rules for success, and an estimated date of completion. The ISP also allows the client to express any cultural preferences or considerations that may need to be taken into account.
