About this file

This Interagency Service Agreement (ISA) Amendment is between the Arizona Department of Health Services (ADHS) and the University of Arizona, specifically involving multiple federally-funded cooperative agreements. The amendment is Amendment Number Two (2) for Agreement Number CTR061468, with Selena Leon as the Procurement Officer. The document encompasses three separate exhibits related to different federal grants: COVID-19 Health Disparities, Workforce Development, and American Rescue Plan Act (ARPA) Emergency Response.

The amendment includes three distinct subawards with different funding amounts and periods: a COVID-19 Health Disparities grant for $1,132,401.31 (total award $34,603,661.00) from May 2021 to May 2024, a Workforce Development grant for $1,280,000.00 from July 2021 to June 2024, and an ARPA Emergency Response grant for $1,980,000.00 from January 2023 to January 2024. All grants are from the Centers for Disease Control and Prevention (CDC), have a 10% indirect cost rate, and are structured as cooperative agreements for public health crisis response and emergency preparedness. The price sheet indicates quarterly invoicing and covers various expenses including staffing, equipment, space adjustments, and oversight, with a total contract value of $2,473,000.00 for the period of July 1, 2023 to June 30, 2024.
