About this file
This document is a Contract Extension Amendment issued by the Arizona Department of Environmental Quality (ADEQ) for Contract No. CTR059035, Amendment No. 2. The contract involves Emergency Environmental Services and is between ADEQ Procurement, located in Phoenix, Arizona, and a contractor providing emergency response and non-emergency environmental services.
The contract extension extends the current agreement to March 1, 2026, with the contractor required to maintain current insurance certificates meeting ADEQ's specified requirements. The amendment authorizes the contractor to continue work until completion of the Task Order and continue services into the new state fiscal year. Invoices for work performed through March 1, 2025, must be submitted by March 31, 2025, and will be paid within 30 days of receipt. The extension maintains all previous contract terms and conditions, with the expectation of a new purchase order to be issued once the budget for the fund account is loaded.
