About this file

This document is a Contract Amendment Summary issued by the Arizona Department of Transportation (ADOT) Procurement division, specifically Amendment No. Six (6) for a contract with Pavement Marking Inc. for Pavement Marking Services. The amendment involves modifications to an existing contract between ADOT and the contractor.

The amendment extends the contract period from August 15, 2025, through August 14, 2026, and makes several key changes to the contract's exhibits and reporting requirements. Specifically, it replaces Exhibit 5 (Pavement Marking Traffic Control Set-Up), removes Exhibits 10 and 11 related to daily traffic control and work reports, and updates the reporting language in Paragraph 14. The new reporting requirements mandate that the contractor provide detailed information upon ADOT's request, including contract and purchase order numbers, location details, job specifics, materials used, quantities, work dates, temperatures, and any unusual activity details. The amendment ensures continued service while streamlining reporting processes for the pavement marking contract.
