About this file
This document is Amendment No. 1 to a Mandatory Use Contract issued by the Ohio Department of Administrative Services (DAS), General Services Division, for a Multi-Agency Maternal and Infant Wellness (MIW) Medical-Legal Partnership (R3). The contract involves the Research Institute at Nationwide Children's Hospital and is distributed to the Ohio Department of Health and the Ohio Department of Children and Youth.
The contract (CSP023160) was originally effective from 08/02/24 to 06/30/25 and has been renewed for an additional 12 months, extending through 06/30/26. The amendment maintains all existing prices, terms, and conditions while updating the contract duration. The contract was awarded following a Request for Proposal (RFP No. SRC0000015829) and is a requirements contract that allows the specified state agencies to purchase contracted services at their discretion, with no guaranteed volume commitment. The contract details and any amendments are publicly available on the DAS website at www.ohio.gov/procure.
