About this file
This document is an Exhibit 1 - Reoccurring QRTP (Qualified Residential Treatment Program) Placement Review form issued by the Arizona Department of Child Safety (DCS). The form is designed to be completed by QRTP providers every 45 days and submitted to the DCS Placement Coordinator and DCS Specialist. It is a comprehensive review document focused on tracking and assessing a youth's progress, treatment, and potential transition out of a residential treatment program.
The form covers multiple aspects of a youth's placement, including progress towards discharge, treatment services, relationship development, school performance, family involvement, and overall program participation. Key sections require detailed reporting on specific trauma-informed services being provided, the youth's adjustment to the program, educational status, engagement with their Child and Family Team (CFT) Service Plan, and efforts to prepare the youth for potential reunification or alternative placement. The document emphasizes ongoing evaluation of whether the QRTP remains the most appropriate and least restrictive environment for the youth, with a focus on supporting the youth's long-term permanency goals.
