Indian Health Service (IHS) VTC digital media upgrade– RFQ-23-PHX-066

TO RECEIVE UP TO DATE INFORMATION ON THIS REQUIREMENT, INCLUDING ANSWERS

TO VENDOR QUESTIONS, MODIFICATIONS TO THE REQUIREMENT OR EXTENSIONS TO THE

OFFER DUE DATE, USE THE FOLLOW THIS REQUIREMENT FEATURE TO ENSURE SYSTEM

NOTIFICATIONS.

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Submit written quotes only; oral offers will not be accepted.

d This requirement is under North American Industrial Classification Standard (NAICS) codes: 334310 - Audio and Video Equipment Manufacturing; 750 employees. This requirement is set-aside 100% Buy Indian (specific to Department of Health and Human Services, Indian Health Services); wherein, Buy Indian means American Indian and Alaska Native Small businesses, in accordance with the criteria and size standards of 13 CFR Part 121.

Definitions. Indian Economic Enterprise (IEE). Any business activity owned by one or more Indians or Indian Tribes that is established for the purpose of profit provided that: the combined Indian or Indian Tribe ownership must constitute not less than 51 percent of the enterprise; the Indians or Indian Tribes must, together, receive at least a majority of the earnings from the contract; and the management and daily business operations of an enterprise must be controlled by one or more individuals who are Indians. To ensure actual control over the enterprise, the individuals must possess requisite management or technical capabilities directly related to the primary industry in which the enterprise conducts business. The enterprise must meet these requirements throughout the following time periods:

a. At the time an offer is made in response to a written solicitation;

b. At the time of the contract award; and

c. During the full term of the contract.

Indian Small Business Economic Enterprise (ISBEE). An IEE that is also a small business concern established in accordance with the criteria and size standards of 13 CFR part 121. Offerors that do not fall into this category will not be considered for award.

REQUIREMENTS: Indian Health Service (IHS) – Phoenix Area Office (PAO), 40 North Central Ave., Phoenix, Arizona 85004-4424 has a requirement to upgrade the current video-teleconferencing (VTC) and digital media capabilities within the Phoenix Area.

The Government intends to enter into multiple Blanket Purchase Agreements (BPA), in accordance with FAR 13.303. A blanket purchase agreement (BPA) is a simplified method of filling anticipated repetitive needs for supplies or services by establishing "charge accounts" with qualified sources of supply. In this case, we aim to establish multiple (2-3) award BPAs. Each BPA holder must provide the following:

1. Provide completed Indian Economic Enterprise Representation form.

2. Authorized reseller or distributor agreement required for the following brands: Cisco and Crestron; the two are Brand Name Only requirements, all other identified products are acceptable as Brand Name or Equal.

3. Identify your Cisco Gold Certified Partner.

A Blanket Purchase Agreement (BPA) does not have any funds committed; it provides a framework for quick Calls to be competed for Tasks on the contract. For instance, at this time, four requirements are being funded and quotes are being sought for those individually (Rooms: 601, 603, 605A & 720). The term for a BPA is five years. During that five years, you can issue Calls against the contract to fund any of the rooms outlined in the SOW, or small, medium or large rooms. Calls are a lump-sum of money used in a declining balance fashion, to fund task completion as provided under the BPA framework (e.g. pricing schedules and terms and conditions). Each Call requirement will be competed among the BPA holders. Given details about the requirement, similar to that as identified in the SOW (specific rooms) or by size standard (S/M/L), the BPA holders shall provide an estimate for project activation (commence site survey phase – equipment lead time – install schedule) in days After Receipt of Order (ARO) using the pricing structures for the Cisco and Crestron products, and the Cisco support.

A pricing mechanism has been provided to allocate costs into three categories: Site Survey, Full Automation Installation and Product Solution.

These categories should bear all costs for each Call. The call will be funded with three (3) line items and the vendor will invoice for services/products under one of those three Contract Line Item Numbers (CLINs). Reference: Statement of Work (SOW).

Site survey consists of sections 1.1.1, 1.1.2, 1.1.3;

Full Automation Installation consists of section 1.1.4;

Product solution consists of section 1.2.

This procurement is for NEW Equipment ONLY; no remanufactured or "gray market" items. Vendor shall be an Original Equipment Manufacturer (OEM authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system, such that OEM warranty and service are provided and maintained by the OEM). All, warranty and service associated with the equipment shall be in accordance with the OEM terms and conditions. All Equipment must be covered by the manufacturer's warranty. The quote MUST include a copy of the authorized distributor letter from the manufacturer to verify that the vendor is an authorized distributor of the products being quoted; failure to provide evidence of this may result in your proposal not being further considered.

This is an open-market combined synopsis/solicitation for products as defined herein. The government intends to award multiple (2-3), Firm- Fixed Price, Blanket Purchase Agreements (BPA) as a result of this combined synopsis/solicitation to the responsive and responsible vendor whose conforming response is determined to provide the best value to the Government, price and other factors considered.

This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2023-04 dated 06-02-2023. It is the contractor's responsibility to be familiar with the applicable clauses and provisions.

CLAUSES INCORPORATED BY FULL TEXT:

52.212-2 Evaluation—Commercial Products and Commercial Services (Nov 2021) The provision at FAR 52.212-1, Instructions to Offerors—Commercial Products and Commercial Services (Mar 2023), applies to this solicitation.

TECHNICAL PROPOSAL: Offerors shall provide a technical quote that includes past performance. Quotes must submit a technical narrative outlining their capabilities for this project; specifically, discuss Brand Name product/service partnerships. A project management approach should be included to discuss an area-wide deployment, receiving at multiple stations and an overall rollout timeline.

Proposed Equipment/Support: The technical quote shall identify the Brand Name items requested. Other product offerings, as identified in the SOW, (not Cisco or Crestron) provide overview of which product brands would be utilized or equal items, accompanied by technical literature with product specifications. Quotes may be evaluated for technical acceptance on a pass/fail basis; items not meeting the technical minimums specified, may be deemed unresponsive and not further evaluated (see Requirements section above). Technical acceptance will be evaluated against the salient characteristics/functions outlined in the document entitled: Statement of Work.

Project management plan: Vendor must communicate level of involvement in the execution of this project; including, planned co-operation between the facility, manufacturer and sub-contractors.

Sub-contracting plan: If subcontracts will be utilized, indicate what aspects of the process will be subcontracted and describe the capabilities of the subcontractor(s); include name of proposed subcontractor(s). Discuss Cisco Gold Certified Partnership.

Narrative response: Vendor must propose plan for Small, Medium and Large room scales to demonstrate magnitude of order, but additionally, flexibility in service unit needs. If invasive construction costs are required, those would be addressed in the design portion of the Call, during the Site Survey phase. All costs will be verified and approved in writing before a Notice to Proceed with the Full Automation Installation phase can be issued to the vendor. Broadly address proposed solutions to mitigate risk and other unforeseen circumstances, based upon your past performance with similar, recent and relevant, projects. Furthermore, any secured pricing on products, and the term should be provided.

Any and all installation, if required, must be done by a Cisco Certified Gold Partner with a minimum of ten years’ experience as a Gold Partner.

RECENT & RELEVANT PAST PERFORMANCE: Offerors shall submit past performance that is current and relevant as defined herein. Offerors may submit a maximum of 5 (minimum of 3) examples of their past performance with their offer to include prime contract and subcontracts. Past performance information should contain the following:

Project title;

Description of the project;

Contract number;

Contract amount;

Government Agency/Organization;

COR’s & CO’s name, address, and phone number;

Current status, e.g., completed and/or if in progress, start and estimated completion dates; and A brief narrative of why you deem the reference to be relevant to this effort.

The Government may also consider information obtained through other sources. Past performance information will be utilized to determine the probability of success relative to the required effort.

Recent is defined as having been completed within the last 24 months from close date of this solicitation. Relevant is defined as any contract or subcontract valued at $500,000 or greater with a similar type requirement.

Relevant Past Performance evaluation via Past Performance Information Retrieval System (PPIRS). PPIRS records for the identified contract will be assessed and other records within PPIRS may be considered. No submission is required for this section.

PRICING: The overarching Blanket Purchase Agreement price will be evaluated in whole, including the following tabs of the excel spreadsheet:

Products – Cisco, Products – Crestron, Services & Maintenance, Small conference room, Medium conference room and Large conference room.

Failure to provide quotes for all of these outlined tabs may be deemed unresponsive and may not be further evaluated.

Vendor must propose product plan to demonstrate magnitude of order, but additionally, flexibility in service unit needs. FAR 52.212-2, Evaluation – Commercial Items.

(a) Evaluation procedures will be IAW FAR 13.106. The Government will select the quote that represents the best benefit to the Government at a price that can be determined reasonable. The Government will perform a comparative evaluation of technical solution, past performance, and price. Technical solution will consist of the following:

Project management plan Sub-contracting plan Narrative response Past performance

Past performance consists of PPIRS and FAPIIS data and relevant past performance. For price, all line items will be evaluated together as a total contract value.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

(d) Comptroller General Examination of Record the Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c) and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause—
