About this file
This document is Amendment No. 3 to AHCA Contract No. MED220, executed between the State of Florida's Agency for Health Care Administration (the "Agency") and Myers and Stauffer, LC (the "Vendor"). The amendment addresses a scrivener's error in a previous contract amendment, specifically correcting the contract's end date specification under Section W of the contract.
The amendment extends the contract's duration, confirming that the contract will begin upon execution by both parties and end on August 31, 2026. The parties had previously agreed through Amendment 2 to renew the contract for three additional years of service and add corresponding funds. This amendment formally corrects the contract language to accurately reflect the intended contract period, ensuring all previous terms remain in effect except for the modified end date.
