About this file

This document is Amendment No. 5 to AHCA Contract No. MED206, an agreement between the State of Florida's Agency for Health Care Administration (AHCA) and Florida State University (FSU). The amendment updates the contract's contact information and replaces Exhibit A-2 with Exhibit A-3, which details a series of deliverables related to a Family Planning Waiver evaluation project.

The contract involves comprehensive research and reporting services for Medicaid data analysis across multiple demonstration years (DY20-DY27, spanning state fiscal years 2017-2025). The vendor (FSU) will provide various deliverables including Medicaid data requests and verifications, survey materials, interim and final evaluation reports, and progress reports. The contract includes specific requirements for data assessment, survey development, and evaluation reporting, with detailed performance standards and financial consequences for non-compliance. The total contract value includes multiple payments ranging from approximately $39,000 to $318,000 for different deliverables, with the amendment specifying precise due dates, evaluation criteria, and financial terms for each project component.
