About this file
This document is Amendment No. 2 to AHCA Contract No. MED206, an agreement between the State of Florida's Agency for Health Care Administration (the Agency) and Florida State University (the Vendor). The amendment modifies an existing contract related to a Family Planning Waiver demonstration project, with the primary parties being a state government agency and a state university.
The amendment adjusts the contract amount to $1,438,966.20, which represents a decrease of $128,324.80, and specifies the Vendor's required non-federal in-kind funding of $1,408,966.20 across multiple state fiscal years (2019-2020 through 2022-2023). The contract involves comprehensive evaluation services, including data requests and verification, survey materials development, interim and final evaluation reports, and progress reporting for the Family Planning Waiver demonstration project. The deliverables are structured across multiple demonstration years, with specific due dates, performance standards, and financial consequences for each deliverable. Notably, the contract includes detailed requirements for data analysis, survey development, and comprehensive reporting to the Agency, with varying payment amounts ranging from approximately $39,000 to $318,000 for different deliverables.
