About this file

This document is Amendment No. 1 to AHCA Contract No. EXD082, an agreement between the State of Florida's Agency for Health Care Administration (the "Agency") and Shutts & Bowen, LLP (the "Vendor"), a legal services contract for the Office of the Attorney General's Private Attorney Services.

The amendment extends the contract term from July 1, 2019, to June 30, 2021, and increases the total contract amount from $150,000.00 to $200,000.00, with an additional $50,000.00 allocated for legal services. The amendment specifies that fees shall not exceed $150,000.00, and reimbursable costs shall not exceed $50,000.00. The vendor is required to notify the agency in writing when billable services reach $125,000.00 and when costs reach $40,000.00, with failure to provide such notification potentially resulting in non-payment.
