About this file
This document is an Amendment to a Standard Professional Services Agreement issued by the State of Florida Department of Transportation (FDOT), District Five, located in Deland, Florida. The amendment involves Ayres Associates, Inc. as the primary consultant, along with two additional firms: Kisinger Campo & Associates, Corp. and FIT Engineering LLC. The original agreement (No. C9V13) was initially executed on 6/22/2017, with this first amendment dated 6/10/2019.
The amendment increases the total maximum limiting amount for services from $665,271.00 to $1,361,552.00, adding $696,286.00 to the original contract. The contract is specifically for District Five Overhead Sign & High Mast Light Pole Inspections (Financial Project No. 412456-1-72-08). The fee increase is distributed among the three consulting firms: Ayres Associates Inc. receives $482,695, Kisinger Campo & Associates, Corp. receives $143,410, and FIT Engineering LLC receives $70,181. The scope of services specified in the original agreement remains unchanged, and the amendment incorporates Attachment No. 1, a Summary of Contract Fees, to formalize the additional funding.
