About this file
This document is a Task Assignment Change Order Form for Contract No. SP708 between the Florida Department of Environmental Protection (DEP) and Eclipse Construction Company. The change order is related to a specific task assignment for work at the Tenoroc site, involving debris removal and road improvement activities. The primary parties are the Florida DEP's Mining and Mitigation Program and Eclipse Construction Company, with Marisa Rhian serving as the DEP Contract Manager and Scott Petro as the Contractor Contract Manager.
The change order modifies the original task assignment by extending the completion date by 11 days, from June 12, 2015 to June 22, 2015, due to heavy rain conditions. The original task assignment value was $30,990, comprised of a $25,545 fee schedule and $5,445 in cost reimbursement. The change order includes a $1,200 additional cost for using a pump to dry the work area, bringing the total task amount to $32,190. The scope of work involves debris removal at the Sand Stock Pile Area, installing culverts, and improving maintenance roads in three separate areas, using bank run shell materials and various equipment such as front-end loaders, dump trucks, and rollers.

Per our discussion, Eclipse wants to complete the work and we are willing to absorb half of the pump cost The new rate is$ 20 per hour and we anticipate using the pump 10 hours per day for a maximum 5 days. With a mob charge of$ 200 This comes to$ 1,200.00. We will hold this at this max, We will need the weekend to get the pump ready for the site and will begin pumping Mon morning if you approve ..

Gerald, After reviewing the $40/hour for the pump and the contract, I don't think we can use the pump. A pump hourly rate wasn't included in our contract. I'm not sure I can justify it as a material. Can someone call me today so we can discuss our options?
I'll be in the field this morning, please call my cell. 863-581-0838

Marisa, It is too wet to work today and I have called off the crew. It will probably be too wet tomorrow also, however Bryan tells me he can possibly bring in sand for a berm and pump starting on Friday.
How do you feel about weekend work?

Area 1 (Debris Removal) Excavate and sort metal debris at the Sand Stock Pile Area and Material Staging Area. Place metal debris into recycle containers. Recycle containers will be placed onsite and removed by another vendor. At the Sand Stock Pile Area, the metal debris is to be excavated from the sand spoil and ground. If Eclipse determines that the metal debris is buried too deep for removal, then Eclipse must notify Marisa Rhian by email, send a photo of the debris, and request approval that the debris may remain in place. If the debris is approved to remain in place, the debris must be cut off at ground level with the chain saw/chop saw and covered over with sand. For safety reason, debris removal at the Sand Stock Pile Area should only occur on Tuesday and Wednesday. Equipment to be used at this Area include: Front End Loader with Operator (40 hours), Pickup with Working Foreman (40 hours), Chain Saw/Chop saw (40 hours)

Area 2 (Road Improvement) Improve a maintenance road by Installing two culverts using corregated metal pipe and laying sand covered by a 6 inch bank run shell road base to the finished grade. The road will be 282 feet long and 14 feet wide. The corregated metal pipe will be provided to Eclipse and cut to the appropriate length. The two culverts will be installed side-by-side at the agreed upon location between Eclipse and Marisa Rhian. The culverts will be installed at the same elevation and not placed to impede drainage. The culverts will be wrapped with filter fabric provided by Eclipse at no cost. The sand will be brought in from the stock-pile. The road across the entire length of the improvement area should have a consistent elevation upon completion; however the road will be slightly slightly slant to the south to allow drainage. Equipment to be used at this Area include: Dump Truck (48 hours), Roller (48 hours), Front End Loader with Operator (48 hours), and Pickup with Working Foreman (48 hours). 8 loads of Bank Shell Run shall be used.

ShellOOl pdf
1. Attached is the last invoice for shell. The actual number per load is $462.00 and markup on top of that. We priced $450 per load before markup.
2. We will wrap the culvert with filter fabric using material left over from a previous job. There will be no cost for the material and labor has been included in the proposal. We have not included any costs associated with the material so we are not including submittals or proof of purchase. If those are required we will have to purchase material and add that to our quotation.
3. Chainsaw was included at $2 per hour. We will provide a chop saw in its place for the same price.
4. I have only eyed the elevation but it appears to me we are fine. If we have to increase the elevation of the fill over the culvert we will make it a gradual transition.

Jeni, I am currently working on the Task Assignment for this work. I have a few questions regarding the
Estimate:
Can you please confirm the cost of the 11 loads of bank run shell is $450 per load? Does this include the materials mark-up?
We had discussed wrapping the culverts on Area 2. Does Eclipse plan on wrapping the culvert? If so, please include in the estimate.
On Area 2, does Eclipse need to include a chain saw/chop saw to cut the culvert?
For Area 2, is there enough of a change in elevation for enough sand and bank shell to be placed over the culverts to allow an even elevation? I want to ensure that we are not left with a hump where the culverts are.
As I mentioned in the field I plan on June 12, 2015 to be the Task Execution Deadline.
I am attaching the Scope of Work, please review and let me know if you have any questions.
