About this file

This document is Amendment No. 5 to Agency Term Contract No. GC856 between the Florida Department of Environmental Protection (FDEP) and ENVISORS-ENSOUTH JOINT VENTURE, LLC, for petroleum contamination site response action services in the South Region. The amendment was executed on April 2, 2014, and involves modifications to the existing contract's rate schedule.

The amendment details rate changes for numerous line items across multiple service categories, including mobilization, drilling and boring, well installation, sample collection, laboratory analysis, soil source removal, remediation system integration, and professional services. Most rate adjustments are increases of approximately 5%, with a few exceptions showing slight decreases. The changes apply only to new work assignments offered on or after the Rate Change Date, and the updated Attachment D (Version 2.2) Rate Schedule supersedes the previous version. The contract governs environmental remediation services, with specific rates for activities such as site assessments, field work, equipment usage, reporting, and professional oversight in the petroleum restoration program.
