About this file
This is Amendment No. 11 to DEP Agency Term Contract No. GC843, a Petroleum Restoration Program contract between the Florida Department of Environmental Protection and BTEX Engineering, Inc. The contract was originally executed on February 25, 2014, and this amendment was signed on August 22, 2022, by John Truitt, Deputy Secretary of the Department of Environmental Protection, and David Chavelo, Project Manager for BTEX Engineering, Inc.
The amendment updates Attachment A (V2.0) of the Scope of Services, specifically Section 12 regarding Debris, Waste, and Product Removal and Disposal. The modification establishes a protocol for handling soil overages during petroleum contamination site response action services. If delivery weight tickets indicate that the volume of soil transported from a site exceeds the approved Purchase Order volume by no more than 20 percent, the contractor must notify the site manager via a Request for Change within two business days of the overage transport. All other contract terms and conditions remain in effect, with this amendment's provisions controlling in the event of any inconsistency with the original contract.
