About this file
Amendment No. 11 to Agency Term Contract No. GC818 is made between the Department of Environmental Protection (DEP), an agency of the State of Florida, and JIM STIDHAM & ASSOCIATES, INC. The original contract for petroleum contamination site response action services was effective February 21, 2014, and this amendment was executed on August 18, 2022, by the contractor and August 22, 2022, by the state.
This amendment updates the Scope of Services, specifically Section 12 regarding Debris, Waste and Product Removal and Disposal. The modification establishes a notification requirement for soil delivery overages: when delivery weight tickets indicate an overage exceeding the volume approved on a Purchase Order, and the overage does not exceed 20 percent of the total volume authorized under the Purchase Order, the contractor must notify the site manager via a Request for Change within two business days of the date the additional soil was transported from the site. All other terms and conditions of the original contract remain in effect, with the amendment provisions controlling in case of any inconsistencies.
