Contract Summary
Amendment No. 12 to Agency Term Contract No. GC781 is executed between the Florida Department of Environmental Protection (DEP) and Universal Solutions, Inc. The contract, originally effective February 27, 2014, pertains to petroleum contamination site response action services under the Petroleum Restoration Program.
This amendment modifies the Scope of Services, specifically Section 12 (Debris, Waste and Product Removal and Disposal) of Attachment A (V2.0) regarding delivery weight ticket procedures. The amendment establishes that when delivery weight tickets indicate soil volume exceeds the amount approved on a Purchase Order, notification to the site manager via a Request for Change is required within two business days, provided the overage does not exceed 20 percent of the total volume authorized under the Purchase Order. All other contract terms and conditions remain unchanged, with this amendment's provisions controlling in the event of any inconsistency with the original contract.
