This document is Amendment No. 13 to Agency Term Contract No. GC763 between the State of Florida Department of Environmental Protection (DEP) and HLM Environmental Remediation, Inc. The original contract was executed on February 27, 2014, and governs petroleum contamination site response action services under the Petroleum Restoration Program. The amendment was signed by James P. Bowen, President of HLM Environmental Remediation, Inc., on August 12, 2022, and by John Truitt, Deputy Secretary of the Florida Department of Environmental Protection, on August 22, 2022.

Amendment No. 13 updates Attachment A (V2.0) of the contract's Scope of Services, specifically Section 12 regarding Debris, Waste and Product Removal and Disposal. All other terms and conditions of the contract remain in effect, with the amendment's provisions controlling in the event of any inconsistency with the base contract.
