About this file
This document is Amendment No. 11 to Agency Term Contract No. GC749, a Petroleum Restoration Program contract executed between the Department of Environmental Protection (DEP), an agency of the State of Florida, and ENVIROTRAC LTD., INC. The original contract became effective on February 24, 2014, and this amendment was signed on August 22, 2022, by John Truitt, Deputy Secretary of the Florida Department of Environmental Protection.
The amendment modifies the Scope of Services detailed in Attachment A (V2.0), specifically Section 12 regarding Debris, Waste and Product Removal and Disposal. The revision establishes procedures for handling soil volume overages during delivery: when delivery weight tickets indicate an overage exceeding the volume approved on a Purchase Order, but the overage does not exceed 20 percent of the total volume authorized under the Purchase Order, the contractor must notify the site manager via a Request for Change within two business days of the date the additional soil was transported from the site. All other terms and conditions of the original contract remain in effect, with this amendment taking precedence in the event of any inconsistency between the contract documents.
