EXHIBIT A‐1
Scope of Work for Monitor Well Installation Contract Collier‐Hogan Project ‐ Collier County, Florida
CONTRACTOR shall install a single interval, telescoping/multi‐cased monitor well at the site located southwest of the city of Immokalee, Florida (the “Site,” Figure 1), to monitor water quality at or near the base of the Underground Source of Drinking Water (USDW). The location of the Site for the monitor well is west of and in proximity to Hughes well 22‐3H (Figure 2(Revision 1)). In addition, the CONTRACTOR shall construct a drilling pad and supply water well to support the construction of the USDW monitor well. The CONTRACTOR’S Work for this project is further described below. In all instances where DEP is entitled to delivery of reports, samples or reserves the right to take or direct actions (including operational activities, suspension of work or the taking of samples), DEP may act through its Contract Manager or may accept such reports, samples or direct any activity through a designated employee, representative, contractor or agent.
1. The total depth (TD) of the USDW monitor well will be either i) approximately 1800 feet below land surface (BLS) or ii) near the base of the USDW, which is indicated in ground water by a total dissolved solids (TDS) concentration of 10,000 milligrams per liter (mg/L), whichever occurs first.
2. CONTRACTOR shall be responsible for drilling site preparation, not limited to, pad construction
(see Figure 2 (Revision 1)), water supply well construction, and the construction of the USDW monitor well and restoration of the site at the completion of this project. CONTRACTOR shall not drill the USDW monitor well below the 1800 foot depth without prior approval from the DEP. The monitor well will be completed with a twelve‐inch diameter open‐hole sampling interval approximately 200 feet in length. All well construction and permitting will comply with all applicable standards and rules established by Florida Statutes, the Florida Administrative Code, the South Florida Water Management District or its delegated authority, DEP (specifically, but not limited to, DEP Underground Injection Control (UIC) rule 62‐528.420, F.A.C.) All costs for insurance, permits, bonds, labor, lodging, equipment, services, materials, supplies, solid waste (garbage) disposal, to appropriately licensed facilities, associated with this project are provided in Figure 3 (Revision 2) of this Exhibit A‐1.
3. CONTRACTOR shall develop a Health and Safety Plan (HASP) associated with drilling and site activities, maintained, and available on‐site, for all personnel entering or working at the site. A copy of the HASP will be provided to DEP for review and acceptance prior to commencement of on‐site activities. CONTRACTOR’s operations shall be OSHA compliant and maintain said compliance.
4. DEP requires appropriate insurance and payment and performance bonding for all Work under the Contract (Exhibit B). Proof of applicable insurance (through a Certificate of Insurance listing the DEP as a certificate holder) will be required prior to DEP’s execution of the contract authorizing the Work to begin.
5. Materials and casing used to construct the well will be new and free of manufacturing oils and grease prior to installation. Lubricants used for drill rods will be non‐petroleum based and approved by DEP.
DEP Contract No. EXC018, Exhibit A‐1, Page 2 of 13
6. Water samples will be collected by DEP or its contractor from the water supply well to be used for drilling fluid make‐up water and grouting purposes. Such samples will be provided to the DEP Central Laboratory for analysis and results to confirm the acceptability of the water quality prior to commencement of drilling the USDW monitor well.
7. Disposal of drilling associated solid waste (cuttings) and fluids (drilling mud), development and testing waste water storage, transport and disposal to appropriately licensed facilities will be the responsibility of the DEP.
8. Utility clearance, if required, will be the responsibility of the CONTRACTOR.
9. CONTRACTOR shall create and maintain a well construction summary report detailing all personnel used and times, drilling progress (to include: problems encountered, footage drilled, down time, casing installation, materials/supplies used, geophysical logging and water quality sampling activities) and provide such report to DEP at the end of each 12 hour shift.
10. CONTRACTOR shall collect a complete set of drill cuttings, washed, lagged relative to depth
(when necessary), and bagged every 10 feet starting at the surface. Each bag of cuttings will be marked with the depth collected from, date, time and the name of the person collecting them.
Each bag will be no less than one (1) full quart in volume. CONTRACTOR shall create and maintain a cuttings sample collection log, documenting the same information as that provided on the sample bag label. An up‐to‐date copy of this log will be provided to on‐site DEP personnel at the end of each 12 hour shift.
11. Monitor Well Design and Construction
The following deep monitor well designations will be used to separate the first well attempted (Deep Monitor Well #1 (DWM‐1)), which was not completed, from the replacement second well (Deep Monitor Well #2 (DWM‐2). This designation will continue throughout Exhibit A‐1.
A. For DWM‐1: The drilling method for the upper 700 feet of the borehole shall be mud rotary.
If zones of lost circulation are encountered any lost circulation materials (LCM) used to regain circulation of the drilling fluid must be approved by the DEP. Drilling below the 700 foot depth shall be by the reverse air drilling method. The source of air being generated from the air compressor(s) used shall have the proper in‐line filtration system to preclude the introduction of oils or contaminates generated by the compressor(s). Two strings of steel, 0.375”–wall, ERW A53B and a third string of steel, 0.028”‐wall, ERW A53B, surface casing will (Refer to Figure 3 (Revision 2)) containing the breakdown of contract pay items.) A minimum of 5.5 inches of over‐drill will be required for the placement of each casing string.
An XY caliper log will be run prior to the installation of each individual casing string. It is anticipated that the Department’s Subject Matter Expert (SME) will contract with CONTRACTOR to require as many as six and no fewer than three interruptions to drilling to determine ground water quality and/or collect confirmatory samples for on‐site testing or laboratory analyses under a separate contract. All cost for this interruption will be the responsibility of the SME under separate contract with CONTRACTOR.
B. For DWM‐2: The drilling method for the upper 1000 feet of the borehole shall be mud rotary. If zones of lost circulation are encountered any lost circulation materials (LCM) used
DEP Contract No. EXC018, Exhibit A‐1, Page 3 of 13 to regain circulation of the drilling fluid must be approved by the DEP. Drilling below the 1000 foot depth shall be by the reverse air drilling method. The source of air being generated from the air compressor(s) used shall have the proper in‐line filtration system to preclude the introduction of oils or contaminates generated by the compressor(s). Two strings of steel, 0.375”–wall, ERW A53B and a third string of steel, 0.028”‐wall, ERW A53B, surface casing will be installed and grouted to surface or in accordance with appropriate rules and statutes. (Refer to Figure 3 (Revision 2)) containing the breakdown of contract pay items.) A minimum of 5.5 inches of over‐drill will be required for the placement of each casing string.
12. The following describes the advancement of the pilot hole for the monitor wells:
A. For DWM‐1: A 12‐inch pilot hole was advanced from the bottom of the second string of casing, approximately 700 linear feet BLS to 935 linear feet BLS. Contractor was unable to complete construction of this well, and terminated its efforts at 935 linear feet BLS.
B. For DWM‐2: A 12‐inch pilot hole will be advanced from the bottom of the second string of casing, approximately 1000 feet BLS to TD. This pilot hole will be used to determine the location of the base of USDW through geophysical logging and/or the collection of in‐situ water quality samples required by the DEP. The last 200 feet (approximately) of the well will be the monitor well production/sampling interval and will be completed as open‐hole.
When the well’s total depth is determined by the TDS concentration, the appropriate suite of geophysical logs will be run to surface or depth indicated by the DEP.
CONTRACTOR shall suspend drilling, when directed, to allow CLIENT to identify through open‐ hole sampling intervals, the transition zone to the base of the USDW. Geophysical logging and packer testing for obtaining in‐situ ground water samples may be required starting at a depth of approximately 1100 feet BLS. The number of water quality tests and type geophysical logging tools required for each event will be determined by the DEP and paid for under separate contract.
13. For DWM‐2: CONTRACTOR shall not commence well development prior to a determination of the TD/sampling interval, installation of the final 6‐inch diameter string of casing and following curing of the grout (a minimum of 12 hours). The well will be developed by pumping until return water is clean/clear and contains a turbidity of less than 20 Nephelometric Turbidity Units (NTUs), has a TDS concentration equal to or less than 10,000 mg/L (or as agreed by DEP), a pH representative of formation water in a limestone unit (approximately 6.9 to 7.5) and is accepted by the DEP. All ground water produced during sampling and well development will be contained in on‐site tanks and removed from the location by an approved transporter and disposed of at a licensed disposal facility at the direction of the DEP under separate contract. Casing strings 1 & 2 will extend to ground surface. The final 6‐inch diameter casing string will extend 3 feet above the ground surface and be protected inside a ten‐inch, steel, above ground vault that will be emplaced in a 3 foot by 3 foot square, concrete pad and secured with a lock. It will be outfitted with a minimum of a 1‐horsepower centrifugal pump. The selection of this pump and associated
DEP Contract No. EXC018, Exhibit A‐1, Page 4 of 13 plumbing will be based on the capacity of the production/sampling interval determined during the water quality collection event and the well development process.
