Hugh Taylor Birch State Park- Restroom & Pavilion Replacements

Additive Change Order No. 1

EAC Project Number 15024.SD0l-02

Dear Ms. Carron:

EAC Consulting, Inc. (EAC) is pleased to submit this fee proposal to provide professional services for the referenced project.

Our services on this project will be supported by the following sub-consultants:

• Justin Architects, P.A., previously known as Cartaya and Associates Architects, P.A. -

Architectural Services

• NVS, Inc. - Geotechnical Services

• Hammond and Associates, Inc. - Electrical and Plumbing

• S&F Engineers, Inc. - Structural Engineering

• Craven Thompson & Associates, Inc. (CTA) - Surveying & Mapping

• Construction Management Services (CMS) - Cost Estimating

Background & Previous Work

The EAC team previously performed a visual condition assessment of four (4) existing building structures located within the Hugh Taylor Birch State Park (formerly approved and executed as

Subtask 1). The findings of the visual condition assessment activities were documented in a report dated, May 2023 and submitted to the FDEP as final deliverable.

The condition of the following buildings/structures were visually assessed (Refer to Exhibit A attached) and are as follows:

• North Restroom - BL039022 (Structure 1)

• Banyan & Live Oak Restroom & Pavilion Combo - BL039014 (Structure 2)

• South BBQ Shelter- BL039015 (Structure 3)

• BBQ Shelter - BL039029 (Structure 4)

The structures were observed to be in generally poor to very poor condition and in need of replacement. The referenced report recommended a pre-manufactured and a concrete masonry unit (CMU) replacement option for each building structure. The feedback received from the FDEP was that final design should be based on the CMU replacement options.
