About this file

This is a Task Assignment Change Order Form issued by the Florida Department of Environmental Protection (FDEP) for work performed by Michael Baker International, a consulting firm. The change order modifies Task Assignment No. 08 under FDEP Contract No. CN202, Project No. 60939, for the Hillsborough River State Park Improve Campsites project. The document was executed on December 18, 2017, with approvals from FDEP representatives including Budget Representative Greg Turral, Project Manager Theresa Carron, Contract Manager Mae Roth, and Contract Authority Michael W. Foster Jr., P.E., along with consultant representative Donnie Bowles of Michael Baker International.

This change order extends the task completion timeline by 122 days without any adjustment to the financial amount. The original task end date of May 31, 2017, was extended to December 29, 2017, and subsequently to April 30, 2018, in order to complete outstanding permit-related tasks. The total task amount remains at $96,950.79, comprising $3,951.75 in time-and-materials charges and $92,999.04 in fixed fees. The extension was necessitated by pending regulatory approvals from the Southwest Florida Water Management District (SWFWMD) for second-phase review and FEMA No-Rise Certification requirements, with no additional cost to the project.

Subject: RE: TA is expiring!

Importance: High

Good Afternoon Teri, After talking with Aaron ... We have a couple things that are in review and out of our hands so to speak.

• SWFWMD is back in for the Second Review and we anticipate another 30 Day review. We should get approval from this submittal.. but as you are aware We cannot Guarantee, that they will not have additional RAls.

• FEMA No-Rise ... This one is the Real wild card as you are aware. Sheila in our Pittsburgh office is currently trying to get some District approval so our report will be an acceptable means in leu of the Full No-Rise Certification.

With this items still left in coordination and waiting approvals ... I think we will request an Extension until April 30th , 2018.

We are hopeful this extension will be enough to close these tasks out and complete this project.

Thank You again for all of your Time and Patience with these Tasks.

Wishing you a Very Merry Christmas and Happy New Year.
