About this file

This document is a Certification Statements form used for bidding on state contracts in Ohio, specifically designed to capture preferences and eligibility for various state procurement programs. The form allows bidders to claim preferences for Domestic Source End Products (Buy American), Ohio-based businesses (Buy Ohio), and Veteran Friendly Business Enterprises (Buy Veteran), with detailed certification requirements under Ohio Revised Code and Administrative Code sections.

The certification document enables bidders to qualify for potential contract advantages by demonstrating their business characteristics, including domestic product sourcing, Ohio economic presence, veteran ownership, and E.D.G.E. (Encouraging Diversity, Growth, and Equity) business designation. Specifically, bidders must provide details about product/service origins, component manufacturing locations, tax status, state registration, employee base, and business classifications. The form serves as a mechanism for state procurement to evaluate and potentially provide preferential treatment to businesses meeting certain economic and operational criteria within Ohio.
