About this file

This document is a Certification Regarding Lobbying form issued by the Nevada State Purchasing Division within the Department of Administration, located in Carson City, Nevada. The form is a standard federal certification that requires vendors to disclose and certify that they have not used federal or other appropriated funds to influence or attempt to influence government officials in connection with federal contracts, grants, loans, or cooperative agreements.

The certification requires vendors to affirm three key points: (1) no federal funds have been or will be paid to influence government officials, (2) if non-federal funds were used for lobbying, the vendor must complete a Standard Form-LLL "Disclosure of Lobbying Activities," and (3) the vendor must ensure similar certification language is included in all sub-awards. The document warns that failure to file the required certification can result in civil penalties ranging from $10,000 to $100,000 for each failure. The form provides blank spaces for the date, solicitation number, vendor name, authorized signer name, and signature, indicating it is to be completed by a vendor as part of a procurement process.
