About this file

This is a Certification of Non-Involvement form issued by the State of New Jersey pursuant to N.J.S.A. 52:32-60.1 (L. 2022, c. 3) for vendors seeking to enter into or renew contracts with state agencies. The certification requires vendors to verify their status on the Office of Foreign Assets Control (OFAC) Specially Designated Nationals and Blocked Persons list, specifically regarding activity related to Russia and Belarus. Vendors must complete one of three certification options: confirming they are not identified on the OFAC list for Russia/Belarus-related activity, acknowledging they are identified on the list due to Russia/Belarus activity, or confirming they are listed but their activity is consistent with federal law, regulation, licenses, or exemptions. The form requires submission of comprehensive vendor information including legal name, Federal Employer Identification Number (FEIN), address, phone number, fax number, and email address, along with the signature and printed name and title of an authorized representative and the date of certification.

The certification applies to all vendors as defined by the statute, encompassing natural persons, corporations, partnerships, limited liability entities, governmental entities and instrumentalities, and any parent, subsidiary, or entity under common ownership or control. The State of New Jersey Department of the Treasury is authorized to impose sanctions, seek compliance, recover damages, declare parties in default, or pursue debarment or suspension for vendors making false certifications. Completion of this certification is mandatory for any entity seeking to establish or renew a contract with New Jersey state agencies for the provision of goods, services, purchase of bonds, or other obligations.
