About this file

This is an attachment document for Request for Proposal Solicitation No. BPM007733 issued by the Arizona Department of Administration State Procurement Office for Employee Behavioral Assessment services. The document specifies insurance requirements that offerors must satisfy as a condition of proposal submission and contract award. Offerors are required to submit a Letter of Insurability from their insurance company demonstrating that they currently possess or can obtain the minimum scope and limits of insurance as detailed in Addendum A of the Special Terms and Conditions. This letter must be clearly marked as "Attachment - Supplement Insurance" in the proposal submission.

Upon contract award, contractors must provide a Certificate of Insurance using standard forms such as ACORD documents along with associated policy endorsements prior to commencing any services or work under the contract.
