About this file
This is a memorandum from the Arizona Department of Health Services' Office of Procurement, dated July 10, 2024, regarding Solicitation No. BPM005681 for the Well Woman HealthCheck Program (WWHP). The document involves three primary contractors: Valleywise-MIHS, Mariposa Community Health Center, and Mountain Park Health Center. The memo is prepared by Jacqueline S. Ortega-Avila, Procurement Supervisor, and approved by Gina Corwin, Chief Procurement Officer.
The solicitation was originally issued on September 1, 2023, with a proposal deadline of September 22, 2023. After awarding contracts to thirteen offerors, it was discovered that three contractors had submitted six-month budgets instead of the requested twelve-month budgets. To address this, the Arizona Department of Health Services is allowing these three contractors to correct their budget workbooks by calculating the total dollar amount by doubling the six-month total. Contract amendments (specifically CTR070059, CTR070060, and CTR070061) will be issued to ensure the correct twelve-month budget is reflected, maintaining fair competition and the best interests of the state.

Regarding: Solicitation No.: BPM005681 Well Woman HealthCheck Program (WWHP)
Subject: R2-7-C318(B) Mistake Discovered After Award
In accordance with R2-7-C318, is has been determined a Mistake has been Discovered After Award. An error was identified after the award for solicitation BPM005681 Well Woman HealthCheck Program.
The RFP was issued on September 1, 2023, with a deadline of September 22, 2023. On January 29, 2024, the Request for Offer Revisions/Best and Final Offer (BAFO) was distributed to all Offerors, due by January 31, 2024. The BAFO and Revised Proposals were then reviewed by the Evaluation Committee on February 1, 2024. During the meeting on the same day, it was noted that further clarifications were necessary due to discrepancies between the amounts in APP and the budget workbooks. Clarifications were conducted via email on February 8, 2024, with a deadline of February 9, 2024, for Offerors to submit Attachment F Budget Workbook. All responses were received on time.
Following the award to thirteen (13) Offerors, it was discovered that three (3) of the awarded Contractors:
Valleywise-MIHS, Mariposa Community Health Center and Mountain Park Health Center. - had only submitted a six (6) month budget instead of the requested twelve (12) month budget for Attachment F – Budget Workbook. Based upon considerations of fair competition and the best interest of the State, the Arizona Department of Health Services procurement department is allowing for a correction to be made to the budget Workbooks for the following contracts: CTR070059 Valleywise Health, CTR070060 Mariposa Community Health Center and CTR070061 Mountain Park Health Center. A Contract amendment will be issued to the three (3) Contractors to correct this mistake. The total dollar amount was calculated by doubling the six (6) month total. Amendment one (1) will ensure that the correct total dollar amount for twelve (12) months is reflected for Valleywise-MIHS, Mariposa Community Health Center and Mountain Park Health Center.
