About this file

This is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania for the Pennsylvania State Police (PSP) Fitness Equipment solicitation. Vendors responding to the PSP's request for fitness equipment must complete and submit this certification form along with their bid by the September 9, 2026 deadline at 11:00 a.m. through the PA Supplier Portal. The form serves as the contractor's attestation of compliance with Pennsylvania's labor and workforce safety laws, and all bids must be submitted electronically through www.pasupplierportal.state.pa.us. Bidders must also complete the Reciprocal Limitations Requirements Act form GSPUR-89 and upload both forms with their bid submission.

The certification requires an authorized contractor representative to verify compliance with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and applicable state labor laws including the Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, and other specified statutes. False certification subjects contractors to penalties including contract termination and three-year ineligibility to bid on future Commonwealth contracts under 62 Pa. C.S. 531. Contractors must immediately report any changes in compliance status to the purchasing agency and acknowledge that false certifications violate 18 Pa. C.S. 4904 regarding unsworn falsification to authorities.
