I. Introduction

The City of South San Francisco ("CITY") is soliciting written proposals from qualified firms ("Consultants") to provide independent special inspection services for Community Civic Campus Phase 3 - Fire Station 63 ("PROJECT").

The PROJECT is located at 71 Camaritas Avenue, South San Francisco, California. It includes demolition of the existing restaurant and construction of a new approximately 10,150-square-foot, two-story fire station with a three-bay apparatus garage, office and living areas, site utilities, public and secure parking, and related site improvements.

II. Scope of Services & Responsibilities

A general description of the required services is contained in this Request for Proposals. The Consultant shall provide all certified personnel, supervision, laboratory services, tools, equipment, travel, coordination, reporting, and closeout support necessary to perform the special inspections and directly associated testing required by the approved Statement of Special Inspections, the applicable Contract Documents, CBC Chapter 17, and the authority having jurisdiction.

III. Background

Fire Station No. 63 is the third phase of the CITY's Community Civic Campus program.

The PROJECT consists of a new 10,150-square-foot, two-story fire station replacing the existing restaurant at 71 Camaritas Avenue. The facility includes office and living quarters, a three-bay apparatus garage, and associated public and secure site improvements.

The structural design includes reinforced-concrete foundations, structural steel moment framing and open-web steel joists, light-frame wood shear walls and diaphragms, reinforced concrete masonry, structural concrete, steel decking, post-installed anchors, and delegated-design architectural and nonstructural components.

IV. Structural Special Inspection Requirements

Structural General Notes Sheet SF0.1 identifies the CBC Chapter 17 special-inspection categories for the PROJECT: steel construction, concrete construction, anchors and dowels in resin, expansion anchors, masonry construction, wood construction, and soils.

Soils and all geotechnical inspection and testing are expressly excluded from this RFP and will be performed under a separate contract.

3 | P a g e

7. Perform required concrete sampling, sample pickup, and laboratory testing, including slump, density, air content, temperature, and compression tests. Include reinforcing tests, aggregate tests, and batch-plant inspection where required by the attached City schedule, subject to its stated waiver provisions.

4 | P a g e

8. Inspect anchors and dowels in resin under Section 03 15 20, including drilling, hole cleaning, embedment, adhesive handling, installation, curing, and installer qualifications.

9. Perform required proof-load testing of anchors and dowels in resin in accordance with ASTM E488 and record tested locations and results.

10. Inspect and test expansion anchors under Section 03 15 30, ASTM E3121, and Sheet SF0.2. Include concrete screw-anchor inspection and required tension or torque testing under Sheet SF0.3 and the applicable evaluation reports.

18. Provide supplemental structural testing for seismic resistance under CBC Section

1705.14 for structural steel.

19. Inspect delegated-design structural and architectural components, including MEP equipment supports and anchorage, where special inspection is required by the drawings, specifications, or approved inspection and delegated-design documents.

5 | P a g e

20. Provide seismic-compliance special inspections and applicable hanger, anchor, and fastener testing for acoustical metal pan ceilings under Section 09 51 33.
