About this file

This document is a name change request letter from EMS LINQ to the Arizona Procurement Portal, dated June 26, 2024, accompanied by a Form W-9. The letter formally notifies the procurement agency of the company's legal entity transition from EMS LINQ Inc to EMS LINQ, LLC, which occurred in July 2022. The company's Federal Tax ID number (82-4127789) remains unchanged, and they have included an updated W-9 form to support this notification.

The W-9 form provides additional details about the company, confirming it is a C corporation located at 2528 Independence Blvd, Suite 200, in Wilmington, NC 28412. The form is signed by the company's Controller on 4/29/2024, certifying the taxpayer identification information and compliance with IRS reporting requirements. No specific contract details, pricing terms, or service descriptions are present in these documents, which appear to be administrative in nature and focused solely on updating the company's legal name in the procurement system.

New line 3b has been added to this form. A flow-through entity is required to complete this line to indicate that it has direct or indirect foreign partners, owners, or beneficiaries when it provides the Form W-9 to another flow-through entity in which it has an ownership interest. This change is intended to provide a flow-through entity with information regarding the status of its indirect foreign partners, owners, or beneficiaries, so that it can satisfy any applicable reporting requirements. For example, a partnership that has any indirect foreign partners may be required to complete Schedules K-2 and K-3. See the Partnership Instructions for Schedules K-2 and K-3 (Form 1065).
