About this file
This is a Worker Protection and Investment Certification Form issued by the Commonwealth of Pennsylvania, which must be completed by contractors and grantees as required by Executive Order 2021-06. The form requires certification that contractors are in compliance with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and thirteen specific state labor and workforce safety laws, including the Construction Workplace Misclassification Act, Minimum Wage Act, Prevailing Wage Act, and various other employment-related regulations.
The certification carries significant penalties for false statements, including contract termination and three-year debarment from bidding on contracts under 62 Pa. C.S. § 531. The form requires an authorized official's signature, printed name, title, and company name, along with the date of certification. This document appears to be related to a District 5 Wastewater Treatment contract opportunity, though it is a standard form that would be required for any Commonwealth contract.
