About this file
This is a Section 504 Compliance Assurance document issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, applicable to contractors bidding on the Human-centered Designer Onsite Support contract for San Mateo Medical Center. The document requires all contractors to certify compliance with Section 504 of the Rehabilitation Act of 1973, as amended, and all associated Department of Health and Human Services (DHHS) regulations. Contractors must acknowledge that contracts will be extended in reliance on the representations and agreements made in the assurance, which binds the contractor, its successors, transferees, and assignees. The assurance differentiates between contractors employing fewer than 15 persons and those employing 15 or more persons, with the latter required to designate a Section 504 coordinator responsible for coordinating compliance efforts. An authorized official of the contracting organization must certify the accuracy of submitted information by signature, title, and date.
The document establishes that contractors with fewer than 15 employees may petition for exceptions to facility accessibility requirements if consultation with disabled persons seeking services reveals no feasible compliance method other than significant facility alterations, in which case the contractor may refer disabled individuals to alternative accessible providers. All contractors must complete and submit the compliance assurance form as part of the procurement process, providing the name of the designated 504 coordinator (if applicable), contractor name, street address or P.O. box, and city, state, and zip code information. This requirement applies to all prospective vendors and is a mandatory compliance condition for contract award in San Mateo County procurements.
