About this file

This document is an Assurance of Compliance form issued by the County of San Mateo Contract Compliance Committee on August 5, 2013, related to Section 504 of the Rehabilitation Act of 1973. The form is a compliance document that requires contractors to affirm their commitment to non-discrimination and accessibility for persons with disabilities. Contractors must indicate whether they employ fewer than 15 or 15 or more persons, and if the latter, they must designate a coordinator to ensure compliance with DHHS regulations.

The form provides an exception for contractors with fewer than 15 employees, allowing them to refer handicapped persons seeking services to alternative accessible providers if making significant alterations to existing facilities is not feasible. The document requires the contractor to provide details such as the name of the 504 compliance person, contractor name, address, and includes spaces for an authorized official's signature, title, and date. This form is part of the county's process to ensure equal access and non-discrimination in contract services.
