About this file

This Bid Form Instructions attachment pertains to the New York State Department of Health's Vaccine Program Supply procurement and establishes the bidding framework for vaccine supply contracts. The Department seeks qualified organizations to provide vaccines for the Children & Adult programs serving Child Health Plus enrollees, underinsured children, and underinsured and uninsured adults through an Invitation for Bids (IFB) issued on August 6, 2026. Bids are due by September 10, 2026 at 5:00 p.m. ET, with written questions accepted through August 13, 2026 at 5:00 p.m. ET and Department responses posted by approximately August 28, 2026. The Department intends to award one or more contracts to ensure all vaccines on the Current CDC Vaccine Price List are covered, with the anticipated contract start date of April 1, 2027. The contract term extends five years from the start date.

Bidders may select from two pricing options for each vaccine line item: Option 1 allows bidders to provide vaccines at prices listed on the Current CDC Vaccine Price List at the time orders are placed, with list prices subject to change throughout the five-year term; Option 2 requires bidders to provide a guaranteed price at or below the current CDC Contract Price List at bid submission, fixed for the contract term. Bidders may mix both options across different vaccine types but must select one option per vaccine line item. All pricing must be based on the CDC Cost/Dose column and include the Federal Excise Tax (FET) of $0.75 per disease component. Bidders may also elect to provide future vaccines added to the CDC Price List by selecting Option 1, provided they bid on at least one vaccine currently listed. There are no MWBE, DBE, or SDVOB contracting goals established for this procurement, and no prior identical or substantially similar contracts have been awarded within the past five years.

Current CDC Vaccine Price List: https://www.cdc.gov/vaccines-for-children/php/price-list/index.html (also referred to as “CDC price list”)

Option 1: The Bidder agrees to provide vaccines at the price listed on the Current CDC Vaccine Price List; at the time an order is placed. The list prices are subject to change over the proposed 5-year term and are established by CDC, independent of the Department of Health. Please refer to the "CDC Cost/Dose" column for relevant pricing.

• Please select "Yes" in Column F for all vaccines your organization is willing to provide at the CDC Contract Price List (“CDC Cost/Dose”). *5

Option 2: The Bidder agrees to provide vaccines at a guaranteed price for the term of this agreement. The guaranteed price must be at or below the current CDC contract Price List at the time of bid submission.

• Please enter a price in column G for all vaccines your organization is willing to provide at or below the CDC Contract Price List (CDC Cost/Dose). *5

1. Bidder's providing a bid price for multiple vaccines may elect to use a mix of both Options 1 and 2, for different vaccine types, but must choose one option per vaccine line item.

a. i.e. A bidder is proposing to provide both Pediatric Vaccines, DTaP and Hepatitis A. They can elect to bid on DTaP under Option 1 (CDC Price list) and select “Yes” in column F, and bid on Hepatitis A under Option 2 (guaranteed price) by entering a dollar value in column G.

2. In the event a bidder erroneously selects "Yes" in column F and provides a proposed guaranteed cost in column G, for the same vaccine line item, DOH may use the cost provided in column G and disregard the selection in Column F or seek clarification from the bidder regarding their intent.

3. Bidders are not required to select "No” in column F or enter a price of $0 in column G for vaccine's they do not intend to bid on. They can simply be left blank.

4. Do Not provide pricing based on the "Private Sector Cost/ Dose" Column of the Current CDC Vaccine Price List.

5. *Please be advised that the CDC Cost/Dose and any guaranteed price entered in column G include the Federal Excise Tax (FET). FET of $0.75 per disease component applies to vaccines covered by the National Vaccine Injury Compensation Program.

Amount of FET applied is referenced in the footnotes of the CDC Vaccine Price List.
