About this file

This is a compliance form attachment for a Request for Proposals issued by the San Francisco Department of Public Health to provide food and nutrition services to low-income HIV-positive individuals in San Francisco. The RFP was published on August 31, 2026, with a pre-proposal conference scheduled for September 10, 2026, at 2:00 PM PT via Microsoft Teams. The deadline for submitting questions is September 17, 2026, with a Q&A addendum expected to post on September 28, 2026. Proposals are due by October 5, 2026, at 2:00 PM PT. Short-listing notifications for interviews are estimated for November 2026, with interviews and the announcement of award also anticipated in November 2026. A Health Commission presentation may follow in December 2026, with an estimated contract start date of April 1, 2027.

This compliance form specifically addresses San Francisco Administrative Code Chapter 12L requirements for non-profit entities that receive a cumulative total of at least $1,000,000 annually in City funds or City-administered funds. Proposers meeting this threshold must complete and submit this form as part of Electronic File #2 and must describe their compliance efforts regarding public access to meetings and records, as well as summarize any substantiated complaints filed with the City within the past two years concerning Chapter 12L compliance. Failure to comply with reporting requirements or material misrepresentation in Chapter 12L submissions constitutes grounds for proposal rejection or contract termination. The form indicates that non-profit entities not meeting the $1,000,000 funding threshold and for-profit organizations are exempt from these requirements.
