1.0 PURPOSE. This Station Standing Order prescribes programs and procedures to safeguard personnel whose research, testing, construction, operating, or maintenance duties require them to work on or in proximity to equipment in which the unexpected energizing, start up, or release of stored energy or contact with energized or moving equipment could cause personal injury or property damage. These programs and procedures are mandated by law and are necessary to prevent personal injury and damage to resources.

2.0 AUTHORITY. The preparation and issue of this Hazardous Energy Control Plan is directed by ER 385-1-31, 01 November 2009, "SAFETY AND OCCUPATIONAL HEALTH, THE CONTROL OF HAZARDOUS ENERGY (Safe Clearance)", Department of the Army, U.S. Army Corps of Engineers.

3.0 APPLICABILITY. The Corps of Engineers' ER 385-1-31 is applicable to all HQUSACE/OCE elements, major subordinate commands, districts, laboratories, and field operating activities (FOA). This policy specifically applies to the dams, spillways, powerhouses, transformer areas, and all of the equipment under the respective operational control of the Electrical Crew Foreman and Mechanical Crew Foreman at the Libby Dam Project. These procedures utilizing lockout will also be used in areas under the control of the Natural Resource Management Staff.

The escorting Affected Person shall write the word “List” next to their name on the Sign-in Sheet to make everyone aware that an Additional Sign-in Sheet has been generated. The escorting Affected Person shall ensure that all visitors signing onto the Additional Sign-in sheet are briefed on hazardous conditions, energy isolation devices, and personal protective equipment. Once all of the visitors have left the Safe Clearance, all visitors shall sign off the Additional Sign-in Sheet and attach it to the original Safe Clearance Sign-in Sheet.

17.7 See Sections 22 and 5.2 for training requirements.

18.0 SAFE CLEARANCE VIOLATIONS.

18.1 Hazardous Energy Control Procedures are for the safety and protection of personnel and equipment. It is vitally important that all personnel subject to Hazardous Energy Control Procedures comply with the intent and scope of this regulation.

18.2 Any person who observes any violation of Hazardous Energy Control Procedures shall inform their supervisor immediately, providing all known facts of the witnessed violation.

18.3 A report of the violation shall be prepared by the witness of the act. The report of the incident shall be submitted to his/her supervisor by the end of the next working day.

The report shall be forwarded to the Operating Project Manager.

18.4 Violators will be subject to appropriate disciplinary action.

19.0 SWITCHING PERMITS.

19.1 Limitations.

(a) Certain equipment as designated by the Responsible Officials may be operated or removed from service by an employee using his/her personally identified Equipment Lockout tag and lock. The Shift Operator shall be contacted in advance to determine that a Safe Clearance is not required, and to determine that other ongoing work is not in conflict. The following conditions will be in effect:

(1) No more than 1 person per Switching Permit (each person must use their own tag and/or lock).

(2) Lockout to be physical.

(3) Switching Permit and any “Abnormal Situation” related to Switching Permit must be approved and recorded by Operator.

(4) No overnight Switching Permits. If overnight is required, will be changed to Safe Clearance.

(5) Switching Permit to cover no more than 1 point.

(6) Personal Protective Grounds shall not be applied to Switching Permits.

(b) Permission to use Personal Equipment Lockout tags will be granted by the Issuing Individual to Authorized Individuals to operate, and/or perform minor maintenance.

(c) All SWITCHING PERMITS will be taken and released during the SAME WORK DAY. A written record of their use will be kept in the Station Log; this record will include the purpose for the outage, the person to whom it is issued and the times of issue and release. The Station Log record will be the only record of the transaction.

(d) The determination as to whether a Safe Clearance or a Switching Permit is required shall be the Issuing Individual’s responsibility. However, the Issuing Individual shall not issue a Switching Permit if the requesting person would rather have a Safe Clearance.

(e) Switching Permits will not be used on equipment under Safe Clearance.

(f) The Issuing Individual and the Authorized Individual requesting to remove equipment from service will agree upon the switching to be performed or equipment to be operated and the points of protection to be tagged with the workman's personal "Equipment Lockout Tag" (and personal lock if the equipment is lockable).

(g) It is critical that all individuals understand that, even though a "formal" Safe Clearance is not required when using a Switching Permit, Hazardous Energy Control measures, i.e. personal lock and/or tag, are required.

(h) Switching Permit Voltage Limitations. Authorized Individuals are permitted to use their Equipment Lockout tags on voltages up to and including 480 volts when authorized to do so by the Shift Operator.

(i) “Switching Permit” work safety requirements. All Authorized Persons working on the same piece of equipment must attach their personal “Do Not Operate” tag and/or lock, identified with their name, at the foremost point of safety prior to performing any work. No exceptions to this rule are permitted. No one is authorized to work under another employee’s personal lock and Equipment Lockout tag. (Refer to Appendix B, Paragraph G for the list of personnel authorized to use Switching Permits).

19.2 Refer to Appendix E for a list of Equipment on which a Switching Permit May Be Applied.

20.0 CAUTION ORDERS.

20.1 Caution Orders are issued to direct attention to abnormal, hazardous, and/or unusual conditions or to special temporary operating instructions. Caution Orders provide protection for equipment only. THEY SHALL NEVER BE USED TO PROVIDE PERSONAL PROTECTION. The Caution Order Tag is to prevent or limit operation, or to provide other special instructions for equipment. The Caution Order Tag NEVER signifies that the equipment is de-energized.

20.2 Each Caution Order shall be assigned a number, using an "X" as a prefix.

Caution Orders shall be numbered consecutively.

20.3 Caution Orders may be requested by any Project employee. They shall be issued by the Shift Operator.

20.4 Caution Orders will be logged in accordance with the procedures outlined for Safe Clearances. The data entered for issue or release will conform to the data required for Safe Clearances.

20.5 A Caution Order Sheet, ENG Form 1928, will be prepared for each Caution Order issued. The completed ENG Form 1928 will be maintained as Project records.

20.6 Caution tags, ENG Form 1924, will be properly completed and attached to appropriate control devices and equipment. The tags will be protected from adverse conditions.

20.7 Releasing of Caution Orders.

(a) Caution orders shall be released and tags removed when unusual or abnormal conditions for a piece of equipment or device have been corrected, and special instructions are not required to protect the equipment.

(b) Generally, a Caution Order should be released by the person who requested it.

However, the Shift Operator, having been satisfied that any unusual or abnormal conditions governing the equipment covered no longer exist, may release the Caution Order himself.

21.0 HAZARDOUS ENERGY CONTROL INSPECTIONS and ASSESSMENTS.

21.1 INSPECTIONS. Periodic inspections shall be performed at least annually by individual(s) knowledgeable in HEC Program and Procedures, to include an Authorized Individual. Personnel internal to the facility may conduct the inspection one year and personnel external to the facility shall conduct the inspection the next year (peer review).

The person using the HEC procedure cannot inspect the procedure. Written inspection reports for all inspections shall include, in a standard format, these items as a minimum:

(a) The clearance number, purpose of the clearance and date on which the hazardous energy control procedure was used.

(b) Names of employee(s) performing the inspection.

(c) Date of the inspection.

(d) Violations.

21.2 REQUIREMENTS FOR INSPECTION. The Responsible Officials shall ensure that periodic inspections of the Project HEC Program are made, and results documented, at least annually, and that the inspections are conducted by Authorized Individuals who are not users of the Safe Clearance that is being evaluated.

21.3 ASSESSMENTS TO BE MADE. Annual Assessments will be performed to:

(a) Check employee knowledge of the HEC program and their responsibilities under it.

(b) Assure all requirements of the HEC program are being met.

(c) Certify that specific systems have been checked.

(d) Assure that the limitations of tags are understood by all Power Plant personnel.

21.4 FORMAT AND CONTENT. All deficiencies found during inspections of clearance procedures shall be documented in the report, and the report shall be signed by the Inspector. The Inspector shall review his/her findings with the appropriate personnel.

The Responsible Officials will certify that all periodic inspections are performed as required at the Project.

22.0 HAZARDOUS ENERGY CONTROL PROGRAM TRAINING.

(3) Methods and means to isolate and control energy;

(4) The requirements of the local HEC Program.

(b) Affected persons shall be instructed in the purpose and use of the HEC procedures. Affected persons must be trained to:

(1) recognize HEC locks and tags;

(2) understand the prohibition against attempting to restart or re-energize a machine, circuit or any equipment that is removed from service by implementation of a HEC procedure; AND

(3) When tagout procedures are used, personnel shall be trained in the imitations of tags,

(c) Incidental Persons. Training for Incidental Persons (visitors and/or other employees who may be in an area where energy control procedures are being used) must include instruction regarding HEC procedures and the prohibition against removing a lockout or tagout device and attempting to restart, reenergize, or operate the machinery.

22.4 Failure to maintain current training status will result in immediate removal from the list of Authorized/Affected Employees. Employees thus removed from the current list shall not be reinstated until they receive annual training and until such training is documented in Project records.

22.5 Retraining shall be accomplished in accordance with the requirements of ER 385-1-31. Retraining is training in addition to initial training and is required under the following circumstances:

(a) At least every twelve months and followed by a written assessment;

(b) When a periodic inspection reveals deviations from procedures or inadequacies in knowledge or use of HEC procedures; or

(c) When there is a change in job assignment, a change in hazards, or a change in the HEC Program.

(d) Whenever inspections reveal, or whenever the employer has other reason to believe, that an employee’s knowledge, understanding, and use of Hazardous Energy Control Procedures is inadequate or deviates from established procedures.
