About this file

This Statement of Work (SOW) details the procurement of eye clinic equipment for the Hershel "Woody" Williams Veterans Affairs Medical Center (VAMC), specifically for the new Prestonsburg Community-Based Outpatient Clinic (CBOC). The acquisition includes multiple essential pieces of equipment for routine eye exams, such as a patient chair, phoropter, acuity chart, and slit lamp, among other diagnostic instruments. The equipment list comprises 28 line items from various manufacturers like Reliance, Volk, Welch Allyn, Reichert, Lombart, and Marco, covering examination chairs, diagnostic lenses, measurement tools, and installation services.

Key deliverables include coordinating with the VA Medical Center's biomedical engineering department for on-site hardware installation, providing product warranties, and ensuring all equipment will be installed and used at the Prestonsburg CBOC located at 236 North Highland Avenue, Prestonsburg, Kentucky. The equipment is critical for establishing a fully functional eye clinic, enabling comprehensive vision assessments including refraction, lens power measurement, eye pressure testing, and various diagnostic procedures. Each item has specific salient characteristics detailing its functional purpose in eye examinations, with most items allowing for brand-name or equivalent alternatives.
