About this file

This is a Drug-Free Workplace Act Affidavit form required by the South Carolina Commission for the Blind for the Summer Teen Program Pre-ETS Services contract opportunity. The affidavit certifies vendor compliance with Section 44-107-10 et seq. of the South Carolina Code of Laws, which mandates maintenance of a drug-free workplace. The form requires vendors to provide standard business information including company name, address, phone, fax, FEIN/SSN, and an authorized signature. Submission of the completed affidavit is mandatory, and failure to furnish it will result in contract delay. The completed form must be returned to the South Carolina Commission for the Blind Procurement Services at 1430 Confederate Ave, Columbia, SC 29201, via phone at 803-898-8814 or email at procurement@sccb.sc.gov.

This affidavit requirement applies to any resulting contract valued at $50,000 or more. The South Carolina Commission for the Blind, which provides quality individualized vocational rehabilitation services, independent living services, and prevention-of-blindness services to blind and visually impaired consumers, implements this drug-free workplace compliance measure as a condition of contract award. No pricing, set-asides, or other financial terms are specified in this particular document, as it serves solely as a compliance certification requirement rather than a standalone solicitation.
